Two sensors are required to find out the direction of rotation. They are placed as shown in the diagram below. One of them is connected to the data input of a D flip-flop, and the other one to the clock input.

If the disc rotates anticlockwise, then the clock sensor goes high first while the data sensor is still low. The output of the flip-flop equals zero. With clockwise rotation, the data sensor goes high first, then the clock sensor goes high. The output of the flip-flop becomes high as well. The timing diagrams are as shown below.
